Admission Criteria:
The case can only maintain limited self‑care ability, i.e., when awake more than 50% of the time, activities are confined to the bed or chair.
There are clear medical and nursing service items that require service.
Cases with chronic illnesses requiring long‑term care or cases needing continued care after discharge.
Target Clients:
Our hospital cases: those who have been hospitalized or treated in outpatient clinics at our hospital.
Non‑hospital patients: telephone consultations, referrals from other hospitals, or self‑referred patients.
Living in Kaohsiung, Fengshan, Daliao, etc., with travel time by car not exceeding one hour round‑trip.
Service Items:
General physical examinations and assessments.
Perform the following according to physician orders:
(1) Medication usage guidance.
(2) Bladder training guidance.
(3) Replacement and care guidance for various indwelling tubes: e.g., nasogastric tube, tracheostomy tube, catheter.
(4) General wound dressing changes, pressure ulcer care.
(5) Assistance with specimen collection.
(6) Subcutaneous and intramuscular injections.
(7) Sputum suction, positional drainage, and steam inhalation guidance.Nutrition and rehabilitation guidance.
Home care consultation and guidance.
Social resource consultation or referral.
