Important Notes:

  1. Please bring your health insurance IC card, ID, child health handbook, household registration book, and pregnancy handbook. After the consultation, please complete the billing on the same day; otherwise, you will need to pay out‑of‑pocket.

  2. 2. To apply for National Health Insurance refunds, please bring the original receipt and health insurance card with related ID documents within ten days (excluding holidays). Outpatient refund hours are Monday to Friday, 8:00~20:00, and Saturday 8:00~12:00.

  3. 3. Patients with scheduled appointments should complete registration at the clinic before the following times: morning session 11:40, afternoon session 16:40, and night session 20:10.

  4. 4. Our hospital offers online first‑visit registration. On the day of the visit, first‑time patients should go to the medical service desk on the first floor (if arriving after 17:00, please go to counters 1 or 2) to complete the “basic information” form before proceeding to the consultation room.

  5. 5. If you need to change your registration number on site, please go to the registration desk before the cutoff time for each session on the day of the appointment (morning session 11:30, afternoon session 16:30, night session 20:00).

  6. 6. Referred patients should bring the referral form and give it to the nursing staff in the consultation room.

  7. 7. Definition of a missed number: “The consultation number is lower than the next number” and “the card‑in time is later than the displayed time for the next number.”

  8. 8. To promote senior‑friendly services, we provide direct consultation for individuals aged 85 and above. When arriving at the consultation room, please first insert the health insurance card to check in and inform the nursing staff (the senior themselves must go to the consultation room). The staff will arrange the consultation according to the situation.

  9. 9. You can use the internet to check the physician’s consultation progress and choose an appropriate time for your appointment.

  10. 10. If the patient is absent but needs medication, please fill out a medication‑collection authorization form at the service desk before 16:30; after 16:30, complete it at the outpatient injection room.

  11. 11. If a diagnostic certificate is needed and the patient is not present, please bring the originals and copies of both parties’ identification documents (copies will be kept in the hospital records) and fill out an authorization form for issuing the certificate (complete it at the service desk before 16:30, or at the outpatient injection room after 16:30).

  12. 12. To ensure patient medication safety and drug quality, self‑pay medications cannot be returned or refunded after leaving the hospital.
