  2. 役男徵兵檢查通知單。(含外縣市役男代檢原縣市通知單)
  3. 近三個月內1吋照片1張。(修圖有明顯落差者不予受理)
注意事項
  1. 全程需更換檢查服及脫襪穿著拖鞋進行體檢。
  2. 專科複檢以本院電話通知或區公所通知單辦理。
報告領取
  1. 無須領取報告。
  2. 初檢：完成體檢隔日起10個工作天後函送縣市政府役政單位
  3. 專科複檢：於完成複檢後，函送縣市政府役政單位。(工作日依檢查項目調整)
