Special employee health check (appointment required): dust, noise, ionizing radiation, high temperature, etc. (For other items, please call for inquiry)
If there are health check items or forms required by your company, please present them at the front desk upon registration.
From November to March each year，morning sessions may have slight schedule changes due to project health checks，please refer to announcements or call for inquiry.

Service Items	General and Special Labor Medical Examination
Check-in Time	Monday to Friday, 13:00 to 15:30 PM
Documents to Bring	國民身分證或健保卡。公司制式體檢表單(視公司需求，若無特殊格式，由本院提供空白表單)。
Important Notices	需空腹滿8小時以上。特殊勞工健康檢查需於1週前預約，請來電洽詢。
Report Collection	完成體檢隔日起5至10個工作天後(特殊項目除外)至本院領取。
Pricing Standards	新進勞工(一般)：950元。在職勞工(一般)：1,200元。特殊勞工、自選項目另計。
