Introduction to Palliative Collaborative Care

“Coordinated Palliative Care” refers to patients in general inpatient wards (excluding admission to palliative care wards) or emergency care who have palliative care needs, and can be referred by the original care team to the hospital’s “Coordinated Palliative Care” medical team to provide appropriate palliative care services.

1. Service Recipients

Patients with end-stage cancer, end-stage motor neuron disease, and end-stage patients of the following diseases, diagnosed or referred by a physician:

  • Dementia, brain degeneration (severe stroke, severe brain injury, multiple sclerosis, Parkinson’s disease, Huntington’s disease)
  • Heart failure, chronic obstructive airway disease, other lung diseases
  • Chronic liver disease and cirrhosis
  • Acute renal failure, chronic renal failure
  • End-stage myelodysplastic syndrome, end-stage frail elderly
  • Patients meeting the clinical criteria listed in the Patient Autonomy Act
  • Rare diseases or other patients with an estimated limited life expectancy

2. Service Items

Symptom Control

  • Provide appropriate medication recommendations for common end-stage symptoms such as pain, dyspnea, nausea, vomiting, abdominal distension, constipation, confusion, weakness, and muscle spasms

Therapeutic Care Procedures and Physical Care

  • Oral care, edema massage, abdominal massage, wound dressing changes
  • Comfort care guidance: repositioning, turning, bedside bathing, foot care, skin care, passive exercises

Psychosocial and Spiritual Care

  • Support and listening, assistance with life review, help in fulfilling wishes, discussion of life’s meaning, restoration of interpersonal relationships

Other Services

  • Referral to home palliative care
  • Assistance with end-of-life preparation and guidance for terminal care

3. Visit Frequency

  • Physician: at least once per week
  • Nurse: at least once per week
  • Social worker and other staff: visits as needed upon referral by nursing staff
