Introduction to Patient Safety Reporting

“Taiwan Patient Safety Reporting System” (Taiwan Patient-safety Reporting system, TPR) was commissioned by the Ministry of Health and Welfare in the 92nd year of the Republic to the Hospital Evaluation and Medical Quality Promotion Association for evaluation and planning, based on five principles of anonymity, voluntariness, confidentiality, non‑punishment, and collaborative learning; it is also non‑mandatory, has no accountability mechanism, and is purely aimed at collaborative learning to prevent errors. It is a nationwide external patient safety reporting system, aiming to foster a positive reporting culture and establish a platform for sharing and communication.

The promotion of TPR not only enables the discovery of errors, analysis of their nature and causes, and the establishment of mechanisms to prevent errors, thereby avoiding the recurrence of the same errors across different institutions or individuals. TPR also serves as an information exchange platform, allowing participants to learn about common types of adverse events among institutions, and by encouraging the reporting of adverse events, it can change the attitude of staff towards errors.

Precautions

  • All reported incidents are serious, urgent, or may lead to medical disputes or legal liability; please handle them promptly according to internal procedures.
  • All reported incidents that fall under mandatory reporting as defined by health authorities or other national laws, such as the Infectious Disease Control Act, should be processed promptly according to legal procedures.
  • The Pharmaceutical Affairs Act requires that serious adverse reactions caused by drugs (medicines, medical devices) be reported to the National Adverse Drug Reaction Reporting Center within the statutory time limit.
  • Needle‑stick incidents involving staff are considered occupational safety incidents; please report them to the Occupational Safety and Health Administration of the Ministry of Labor.
