Taiwan is an aging society with a growing population of chronic disease patients, and sarcopenia has become an important risk factor affecting mobility and quality of life, as muscle strength declines, endurance decreases, and fall risk increases. Early screening and intervention for sarcopenia can effectively delay functional decline.

Our team promotes an integrated care service model for sarcopenia, extending services from medical institutions to the community to establish a systematic service process, including preliminary sarcopenia screening, functional assessment, risk stratification, and intervention follow‑up mechanisms.

Those assessed to be at risk of sarcopenia may participate in a three‑month exercise program, guided by professionals in resistance training and functional exercises, with regular follow‑up.

If the assessment results indicate a need for further medical care, we will assist in referring to hospitals for integrated outpatient services and professional team case tracking to establish a complete care service chain.
