Purpose: Recruit one Heart Failure Case Manager (HF-PAC)
Department: Cardiology
1. Examination Eligibility:
1. Must complete and pass the Taiwan Nursing Association N3 Clinical Professional Competency Training.
2. Clinical nursing experience must include at least one of the following
(1) Cardiology ward ≥ 3 years
(2) Cardiology ICU ≥ 2 years
(3) Hold a specialist nurse license or have completed a specialist nursing training program recognized by the central authority
and have cardiology ward or ICU ≥
2 years of clinical nursing experience.
3. Must complete the Core Course on Acute Late‑Stage Care for Heart Failure (course duration 18~20 hours)
2. Examination Method: Document review (50%), oral interview (50%)

4. Job Responsibilities
1. Responsible for integrating resources of the multidisciplinary medical team for HF‑PAC care.
2. Inpatient care:
(1) Screen cases.
(2) Provide personalized education on heart failure knowledge and self‑care implementation, jointly develop
the required disease care goals.
(3) Conduct training on self‑care abilities and the use of related documentation tools.
(4) Supervise and follow up on required follow‑up tests and examinations, set the target weight range for patients.

(5) Based on the case and family resource situation, consult a social worker as needed to assess available
care interventions and resources.
3. Discharge preparation content:
(1) Before discharge, confirm the patient has prepared self‑care equipment.
(2) Home visit for the case
(3) Recommendations for subsequent exercise training therapy
(4) Cardiac treatment planning: based on the case condition, recommend subsequent cardiac catheterization, cardiac rhythm
devices, defibrillator, ventricular assist device, cardiovascular surgical procedures.
4. Regular assessment and health‑insurance bureau reporting
5. Assist the hospital in collecting and organizing medical evaluation data.
6. Assist the head of cardiology with related tasks.
5. Benefits:
1. Employed staff position, starting salary is Employment Grade 1 (33,720 NT$), and can be gradually promoted based on
years of service and evaluation results, up to Employment Grade 2.
