1. Examination Qualifications:
1. Graduated from relevant public or private universities at home or abroad, with a medical/nursing practitioner certificate.
2. At least one year of clinical care work experience.
3. Experience in clinical care and education for asthma and chronic obstructive pulmonary disease is preferred, especially with case management experience.
4. Holding the “National Health Insurance Asthma Chronic Care Qualification Certification” and the “Chronic Obstructive Pulmonary Disease Care Qualification Certification” is a plus.

5. Job Content:
1. Track patients' disease knowledge and caregiving behaviors, strengthen the knowledge of patients and families as needed, and arrange follow-up examinations and treatments.
2. According to physician orders, assist in arranging early follow-up visits or facilitate related medical care as directed by the physician.
3. Provide telephone consultations and monitor the implementation of patients' home care.
4. During follow-up visits, case managers assist in reviewing the patient's home self‑care records, strengthen self‑care abilities, provide medication guidance according to physician orders when drug types or dosages change, and refer to a pharmacist consultation clinic when necessary.
5. Based on case assessment results, horizontally coordinate and integrate multidisciplinary integrated care.
6. According to case needs, vertically communicate to integrate patient information, provide medical consultation, and arrange referrals across regions and medical institutions.
7. Internal medicine department disease-specific certification quality program tasks, assisting with pre‑review drug applications for pulmonary diseases.
8. Ad hoc tasks assigned by the unit supervisor.
6. Benefits:
1. Adopt a contract employment staffing system, with an entry salary of Grade 1 (33,720 NTD), and can be gradually promoted based on years of service and evaluation results, with the highest salary grade reaching Grade 2.
