Purpose: Recruit 1 Heart Failure Case Manager (HF-PAC) 

Department: Cardiology 

 

1. Examination Eligibility: 

  1. Must complete and pass the Taiwan Nursing Association N3 Clinical Professional Competency Training. 

  1. Clinical nursing experience must include at least one of the following 

   (1) Cardiology ward ≥ 3 years 

   (2) Cardiology intensive care unit ≥ 2 years 

   (3) Hold a specialist nurse license or have completed a specialist nursing training program recognized by the central authority 

      with a training course certificate, and have clinical nursing experience in a cardiology ward or ICU ≥  

      2 years of clinical nursing experience. 

  1. Must complete the Core Course on Acute Post-Phase Care for Heart Failure (course duration 18~20 hours) 

2. Examination Method: Document review (50%), oral interview (50%)

4. Job Responsibilities 

  1. Responsible for resource integration of the multidisciplinary medical team for HF-PAC care. 

  1. Inpatient care: 

(1) Screen cases. 

(2) Complete personalized education on heart failure‑related knowledge and self‑care implementation, jointly draft 

     Establish the required disease care goals. 

(3) Complete training on self‑care abilities and the use of related recording tools. 

(4) Supervise and monitor required follow‑up tests and examinations, and set the weight range the patient should maintain 

     range. 

(5) Based on the case and family resource situation, consult a social worker as needed to assess available 

     care interventions and resources. 

  1. Discharge preparation contents: 

(1) Before discharge, confirm that the patient has prepared self‑care equipment. 

(2) Home visit for the case 

(3) Recommendations for subsequent exercise training therapy 

(4) Cardiac treatment plan: Based on the case condition, recommend subsequent cardiac catheterization, cardiac rhythm 

     devices, defibrillator, ventricular assist device, cardiovascular surgical examination and treatment. 

4. Regular assessment and health insurance bureau reporting 

5. Cooperate with the hospital in collecting and organizing medical evaluation data. 

6. Assist the head of the Cardiology Department with assigned tasks. 

5. Benefits: 

  1. Recruitment under an employment system, with an entry salary of Employment Grade 1 (33,720 NTD), annual salary about 

    550,000, and can be gradually promoted based on years of service and evaluation results, with the highest salary grade reaching Employment Grade 2 

    etc.

  1. Includes year‑end bonus, labor benefits, health insurance, and annual employee health examinations. 

  1. After six months of service, eligible for a 3‑day special leave, with vacation days adjusted according to seniority (in accordance with the Labor Standards Act
